RESPONSIBILTIES
- Connect with individuals who have submitted requests for information
- Lead structured conversations to understand each person’s situation and priorities
- Walk through available options in a clear, simple way
- Help individuals determine next steps based on their needs
- Assist with completing applications and required steps
- Maintain consistent follow-up communication and manage a pipeline of active conversations
- Track progress and keep detailed records
