Remote Nurse Practitioner

REQUIREMENTS

• Active, unrestricted NP license in both New York or New Jersey (preferably both) .

Preferred :

• Active DEA and NPI registrations.
• Prior familiarity with care coordination models (such as CCM, PCM, or RPM).
• Access to a quiet, HIPAA-compliant remote work setup with reliable internet.

RESPONSIBILITIES

• Conduct virtual telehealth intake visits to assess patient eligibility for clinical programs.
• Formulate and document initial care plans addressing chronic conditions.
• Perform initial Social Determinants of Health (SDOH) screenings.
• Ensure accurate documentation aligned with program commencements.
• Provide a seamless clinical handoff to our internal navigation team.
