Medical Records Coordinator
Job Description
REQUIREMENTS
- High School Diploma or GED
- 1–2 years of administrative or medical records experience preferred
- Experience working with the frail or elderly population is a plus — we’ll provide training as needed
- Successful completion of background check, physical, drug screening, TB Screening
RESPONSIBILITIES
- Establish and maintain accurate medical records for all participants
- File and request medical records from external providers
- Maintain participant data for program reporting and evaluation
- Create and update authorization for ER stays and Skilled Nursing facilities
- Participate in team meetings and quality improvement initiatives
- Follow all procedures to ensure accuracy, compliance, and consistency
