Clinical Supervisor
Job Description
REQUIREMENTS
- Bachelor’s degree in Nursing (BSN) preferred
- Active unrestricted Registered Nurse (RN) license in the State of Ohio (multi-state compact license preferred if applicable)
- Minimum 5 years of clinical nursing experience; preferably in palliative care, hospice, or value-based care settings
- Minimum of 2-3 years of supervisory or leadership experience overseeing clinical teams
- Demonstrated ability to manage interdisciplinary teams and performance
- Knowledge of value-based care principles, population health, and chronic disease management
- Skilled in data-driven quality improvement and performance monitoring
- Demonstrated leadership skills and ability to inspire, motivate, and influence others
- Excellent interpersonal and communication skills, negotiation, relationship-building, and problem-solving skills
- Strong leadership, coaching, and conflict resolution skills
RESPONSIBILITES
- Provide direct supervision, coaching, and performance oversight to field-based clinical staff, fostering a culture of accountability, teamwork, and excellence
- Ensure compliance with all regulatory, accreditation, and organizational standards, policies, and procedures
- Coordinate and oversee patient care activities, collaborating with interdisciplinary team members to promote optimal clinical outcomes and patient satisfaction
- Manage staff scheduling to ensure appropriate coverage and continuity of care across assigned territories
- Facilitate and support staff onboarding, training, and ongoing professional development, promoting adherence to evidence-based practices and value-based care principles
- Address patient and family concerns promptly and effectively, advocating for quality care and service excellence
- Monitor key performance metrics, clinical quality indicators, and operational goals; implement targeted improvement initiatives as needed
- Actively participate in departmental and organizational meetings, contributing to quality improvement efforts, workflow optimization, and strategic initiatives
- Ensure timely and accurate completion of documentation and records in accordance with legal, ethical, and organizational requirements
- Maintain current knowledge of nursing standards, clinical best practices, and healthcare regulations relevant to value-based and community-based care models
- Serve as a liaison between field staff and health plan care managers to ensure coordinated, patient centered care delivery
- Verify and ensure proper documentation of all patient interactions and interventions
- Participate in after-hours call schedule via phone
- Perform other related duties assigned to support operational and clinical excellence
