REQUIREMENTS
Must be currently Certified: CMA, CCMA, or RMA Certifications
- Ability to follow HIPAA guidelines.
- Handle potentially difficult conversations in a calm and professional manner.
- Previous F2F healthcare experience required.
- Previous phone experience preferred.
- Compensation range $20-$26 per hour with incentive.
RESPONSIBILITIES
- Verify and record the patient’s demographics, medical issues, medications, goals, barriers, and allergies to create a clinical summary using our certified software program.
- Create a patient-centered, comprehensive plan of care for all health issues based on a physical, mental, cognitive, psychosocial, functional, and environmental (re)assessment, and an inventory of resources.
- Help provide the patient with a means to make timely contact with health care practitioners in the practice who can address any urgent chronic care needs. Ensure continuity of care with their designated practitioner by setting and reminding of regularly scheduled visits.
