Care Coordinator

Job Description

REQUIREMENTS

3–5 years of experience in clinical or community-based care settings (reduction considered for licensed professionals)

Strong organizational skills and ability to maintain accurate documentation

Comfort using communication technologies and electronic systems

Knowledge of local healthcare and community resources is a plus

Valid driver’s license and reliable personal vehicle required

RESPONSIBILITIES

Promote timely access to appropriate medical and supportive care

Develop, implement, and monitor individualized care plans

Coordinate communication between clients, families, hospitals, clinics, specialty providers, and community partners

Manage transitions of care and referrals

Support personal care needs (groceries, personal care items, durable medical goods)

Maintain complete and accurate electronic records

Identify at-risk clients and contribute to program improvement efforts

Reassess client status and adjust care plans as needs evolve
