Job Description
REQUIREMENTS
Experience:
Prior clinical or direct patient care experience preferred but not required. Proficiency with Microsoft programs (Excel, Word, Outlook, Teams, etc.) preferred. Professional communication and teamwork required.
Education:
High School education or GED required. Must be comfortable using computers and learning new technology.
RESPONSIBILITIES
- Coordinate deployment of home-based care services as appropriate (home health, paramedicine, urgent/emergent EMS response) with RPM team members.
- Access, navigate, and document as appropriate in the EMR System.
- Maintain patient privacy per HIPAA guidelines.
- Act as a point of contact for patients enrolled in the Chronic Care Management program.
- Monitor alerts and check for missing data in a timely manner
- Assist patients with set up and troubleshooting for remote monitoring equipment
- Receive and relay patient questions/concerns, and coordinate care as appropriate per established evidence-based protocols.
- Schedule follow up appointments for the patient as needed.
- Provide feedback to the appropriate care coordination staff members regarding patient interactions.
- Collaborate with Primary Care, Care Coordination, EMS crews, Pulse Center, and other specialists to meet patient needs.
- Participates in performance improvement activities to enhance service and patient care.
