REQUIREMENTS

• Licensure:
• Medical Assistant (CMA),
• Experience: Previous experience in primary care, care coordination, or chronic disease management is generally preferred.
• Skills: Strong communication skills, empathy for high-risk patients, familiarity with Electronic Health Records

RESPONSIBILITIES

• Care Planning & Assessment: Develop, maintain, and update a comprehensive care plan, including a problem list, expected outcomes, measurable treatment goals, and symptom management.
• Care Coordination: Manage referrals, facilitate communication between providers, and coordinate with community resources.
• Patient Engagement: Conduct monthly phone interactions with patients to manage conditions, check on health status, and provide education on self-management.
• Medication Management: Reconcile medications, ensure adherence, and manage refills.
• Documentation: Accurately document all CCM services in a Certified Electronic Health Record (EHR) to comply with Medicare billing requirements.
