REQUIREMENTS

  • High school diploma or equivalent required; associate’s or bachelor’s degree in healthcare administration, public health, or a related field preferred
  • Experience in patient access, healthcare administration, or customer service preferred
  • Familiarity with EMRs, patient portals, or scheduling systems
  • Excellent communication, problem-solving, and multitasking skills
  • Detail-oriented and able to adapt to a fast-paced, remote environment
  • Bilingual (Spanish, Mandarin, or other languages) preferred but not required

 

RESPONSIBILITIES

  • Review patient records in electronic medical record (EMR) systems to identify eligible patients for quality care interventions
  • Verify and update demographics, insurance, and medical information
  • Contact patients by phone, text, and email to provide education on recommended screenings and preventive care
  • Schedule appointments and document all outreach activities in Caret Health’s internal systems
  • Coordinate with the Caret Health team to meet program goals and maintain high-quality patient engagement
  • Support evolving workflows related to value-based care initiatives
