REQUIREMENTS

  • Experience in healthcare administration, records management, scheduling, or billing preferred.
  • Knowledge of home health documentation or Medicare guidelines is a plus.
  • Strong attention to detail with the ability to manage multiple priorities.
  • Excellent professional communication and follow-up skills.
  • EMR experience preferred (WellSky, AxisCare, or similar).
  • Must comply with required continuing education including Corporate Compliance and OSHA/Infection Control. Additional education may be requested to support performance goals.

 

RESPONSIBILITIES

  • Reviews incoming documentation for completeness and accuracy according to established guidelines.
  • Prepares PCR submissions within required time frames and payer rules.
  • Follows up promptly with team members to resolve missing or incorrect documentation.
  • Tracks F2F documentation from referral to receipt of signed provider certification.
  • Confirms encounter details support homebound status and skilled service requirements.
  • Contacts physician offices to secure corrections or required signatures.
  • Uploads all required documents accurately to the EMR.
  • Monitors outstanding items and communicates priorities to appropriate personnel.
  • Assists with audits, billing validation, and documentation requests.
  • Works closely with Intake, Scheduling, Billing, Community Care Coordinators, and Clinical leadership to support documentation workflow.
  • Communicates professionally and proactively with Community Care Coordinators to clarify documentation needs and resolve concerns impacting care.
  • Maintains positive, effective communication with physician offices and referral partners to support timely submission of required forms.
  • Provides internal updates with clear expectations and next steps.
  • Ensures documentation practices align with Medicare, payer, and internal policies.
  • Maintains strict confidentiality in compliance with HIPAA.
  • Identifies and addresses potential documentation risks or trends.
  • Participates in meetings, training, and workflow updates as assigned.
  • Partners with Community Care Coordinators to improve communication efficiency and prevent delays caused by missing paperwork.
  • Supports quality improvement initiatives that enhance documentation performance.
